Daily vs On-Demand Tadalafil: Which Should You Choose?
Neither pattern is best for everyone. Compare daily low-dose and on-demand tadalafil and see which suits your sex life and health.

Key points
- Daily tadalafil (2.5–5 mg) keeps you ready without planning; on-demand (10–20 mg) is taken only when needed.
- Daily 5 mg also helps urinary symptoms of an enlarged prostate; on-demand suits men who have sex less often.
- With either pattern, never combine with nitrates or poppers; on a daily dose the drug is always in your system.
Many patients ask me, "Would taking it every day be better?" or "If I take it daily, will my body get used to it?" Both patterns have their place. Neither is better for everyone; it depends on how you live and what other health conditions you have.
Two ways to use tadalafil
Tadalafil is a long-acting PDE5 inhibitor, so it can be used in two ways, which is harder with the other drugs in the group. Read the basics in Tadalafil (Cialis): how it works.
| Topic | Daily use | On demand |
|---|---|---|
| Usual dose | 2.5–5 mg once a day | 10–20 mg before sexual activity |
| Timing | Same time every day, with or without food | At least 30 minutes to 1–2 hours before |
| Readiness | Ready at any time after about 5 days of continuous use | During the active window, up to 36 hours |
| Enlarged prostate | 5 mg helps urinary symptoms | Not used to treat the prostate |
| Side effects | Usually mild because of the low dose, but can occur every day | Stronger per dose, but only on days you take it |
Daily
Daily use
- Commonly 2.5-5 mg once a day
- Ready at all times after about 5 days
- 5 mg also treats enlarged prostate
- Side effects usually mild, but possible daily
On demand
As needed
- Commonly 10-20 mg before sex
- At least 30 minutes to 1-2 hours before
- Works for up to 36 hours
- More side effects per dose, only on days used
Dose and pattern are as assessed by your doctor.
Benefits and limits of daily use
Benefits
- No planning or timing; intimacy can happen naturally
- Removes the stress of "waiting for the pill to kick in", which helps a lot for men with erection anxiety
- Treats urinary symptoms from an enlarged prostate (BPH) at the same time
- Some research suggests it may help the function of the blood vessel lining, but the evidence is not strong enough to say it "restores blood vessels" or cures ED
Limits
- You have to take it every day; forgetting often gives inconsistent results
- Side effects such as headache or back pain can occur even on days without sex
- Monthly cost may be higher if you don't have sex often
- Because the drug is always in your system, you must be careful about nitrates at all times
Benefits and limits of on-demand use
Benefits
- Only taken on days you want it; suits men who have sex less often
- A clear effect during the window you need
- On days you don't use it, there is no drug in your body
Limits
- You need to plan ahead; forgetting or taking it late can mean missing the moment
- Some men find it feels unnatural, or their partner feels everything has to be planned
- The dose per occasion is higher, so side effects per occasion are greater
Who suits which pattern
- Sex twice a week or more, or wanting everything to feel natural: daily use is often a good fit
- Frequent urination or feeling you haven't emptied your bladder because of an enlarged prostate: daily use gives two benefits at once. Read about health at this stage of life in Men's health after 50
- Sex less often or in phases: on demand is better value
- Significantly reduced kidney function: daily use is usually not recommended, and the doctor will choose a safer option
Some men start with one pattern and switch to the other as their life changes. If you have questions about on-demand doses, see Tadalafil 10 mg vs 20 mg.
You can switch patterns as your life changes.
Original brand vs generics: is there a difference?
The original brand (Cialis) and the generics registered in Thailand contain the same active drug, tadalafil, and must pass bioequivalence testing to show they are absorbed in a similar way. The main differences are the brand, the look of the tablet and the price. The choice therefore comes down to convenience and budget, with a doctor's assessment. What you should avoid is unregistered medicine or medicine sold through channels you cannot verify.
Precautions for both patterns
- Tell your doctor about all your medicines, including blood pressure medicines, prostate medicines and HIV medicines
- Go to hospital immediately if an erection lasts more than 4 hours
- Have regular health check-ups, because ED can be linked to diabetes and high blood pressure

Frequently asked questions
If I take it every day, will my body get used to it so I need a higher dose?
This kind of tolerance is not generally seen. If the effect fades over time, it is usually because an underlying condition such as diabetes or vascular disease has got worse, so see your doctor for a review.
I take it daily and missed a day. Should I take an extra dose?
No, don't double up. Take your next dose as usual, and never more than once a day.
Can I stop daily tadalafil?
Yes. It is not addictive, but talk to your doctor first to plan whether to switch to on-demand use or how to continue your care.
